 I was practicing with the SB600. The eyes are just a tad off of tack sharp but close. It was a solo evening so I had to make adjustments and get back in front of the camera.

RAW Conversion
Selective color with mask to drop the background a little bit
Black and white
Brightness/Contrast for eyes
Curves
Border
Save to jpg
Save for web

The fact that you have parts out of focus that would normally be in focus seems appropriate for what I consider to be the homeless look of this image.
