The man is wafting incense smoke into his (and consequently, his son's) face. The boy didn't appear to enjoy the experience, seeming to struggle a bit during this ritual performed by those who wish to receive the blessings and curative powers of the smoke (which is sometimes referred to as the breath of the Gods).
