Good image overall and a good use of selective desaturation. However, the sadness isn't conveyed as well as some of the other images in this challenge involving pills. Good shot, nonetheless.
While this does convey emotion and the photo is well taken, it lacks originality. I'd suggest spending more time exploring different ways to convey a meaning.
Just a thought: I think it might have been more dramatic to have the pills in the foreground, and the person throwing his hand up to his mouth, somewhat blurred in the bkg.
