Levels adjusted, saturation boost, overall burn, scalpel as new layer, background was desaturated with a sepia photo filter and blurred slightly with Gaussian.

I like the way the light strikes the scalpel, but probably needs more of it. The forceps almost blend into the bg, so are probably more of a distraction. A tighter shot would have more impact.
