Now this was simple but I messed up and time constraint prevented the final tweak. Used two sheets of ivory shaded sketching paper. I have not sketched in years but the bug to do this image bit me. The original is wonderful and it is not as popular though it leaves a definite impression once seen. I would expect the original to have had more views then it does.
Anyway, I sketched the finger first by holding finger behind paper against the light to get the proportion right. I then sketched the finger and then the big job of the pencil. You put one paper over finger (the one with the sketch) and the other on the bottom and you light as best as you can. You need a deep dof because one paper is higher then the other and you need to get close to the texture.
The blending can not be done with the clone or the healing brush. You could but you will lose so much detail and then the textures will not match. Adance editing comes to the rescue. Here I employed a layer mask with a gradient, so I was forced to clone the color of the finger up. The layer mask with the sketch, then lies over the finger as it grows transparant in increments. This leaves you a tiny amount to clone to perfect.
My error was in the take. I sketched the finger in one angle with mental note to observe it and then just stuck the finger in without orientation. I then received a photo assignment and was unable to get back and reshape the finger which is no big deal. I was prevented from returning. 20 minutes left to deadline and instead I spent it making comments on another challenge.

good work, Dan.
Yes, on closer inspection, the angle of the finger *goes* look rather weird. I then read your description and understood what happened. But still, your creativity always left me in awe. Thanks for sharing your ideas and methods.
Looks like the hand of Dan again :) One small critisism: The drawn finger looks turned flat up and the real finger apears turned a little right. Maybe I'd notice more cause I study and draw anatomy. Otherwise very cool!!!! This done like the 'Wine back to Water'? Wish I could do that but my camera is very limited. Good luck!
Outstanding... Of course, I would expect nothing less from the master (at least I think I recognize that hand!). ;-) Beautiful work. Whether or not this is Dan, please take it as the compliment it was intended.
