Props dept., the girl, helped out with the....props. She took her top off volunteerally for this pix. She also let me use her 7.7 MP Nikon point and shoot for this effort.
The doctor, a set lighting tech, helped as well.
I used some of the lights from my department to light my image.
Nothing in this pix is real. The needle is retractable, the blood is motion pix quality.
PS= No crop, Levels sampleing, burning, contrast adjust, USM, high pass with hard light, adjust the highlights and shadows.

It's even more mad when you're the patient! My brother-in-law has undergone something like this a few times to have his bone marrow checked for malignant cancer cells. Straight through the breast-bone, I imagine the needle is bigger though. He passed out the first time, now he requests a general anaethestic for the procedure.
This is pretty close to how I picture it.
Good image in anycase.
No offense intended, but you can tell this isn't a real doctor. The first thing I thought of when I saw this pic is "why is the doctor trying to use a syringe to go through a half inch of solid bone?
Most people are aware that the center plexus region is a meeting of many bones and heavy cartilage in the ribcage creating a really tight barrier for a syringe, even one as veterinarian as that.
It might have had better effect if you had left the blood (albeit a little more of it, I thought that was a shadow at first glance) and switched the syringe for a power drill. Alternatively, you could have moved the syringe over to a place where the rib cage has some gaps.
A couple of taps on your own ribcage would be useful here.
I actually like the choice of girl and her bra. It adds a bit of realism in that she doesn't look too perfect. She looks plainly like a typical hospital patient.
well, i like the model she looks like she is asleep the probs are good, unlesss you really are a surgen of sorts, still im not sure what the illusion is, is that blood in the middle of her chest, i guess that could be the illusion, the light takes away fromt eh needle, cant really see it,
a cool idea, just needs something. can't really pick it out. it may be that ultra bright light that hurts the shot. i know its supposed to simulate the doctors light beam, but i think it just detracts from the shot.
Pretty model. I don't quite get the rest :) The hotspot is a bit blown out, and I think this photo is begging for trhe dark, desaturated, grainy treatment.
I get the illusion, however, it is really hard to see details with the action so far away and the spot light so bright. The bra is also kind of distracting. Perhaps using a man for the patient model would have been better.
