 Two speedlights with shoot through umbrellas. One almost directly above the camera and the second from the back right. Silver reflector on lap to help fill in shadows.

What smardaz said, also, google Peter Hurley and watch some of his head shot videos on you tube ;) (I got the same suggestion earlier in the month and found it well worth my time)

looks like you put yourself on a different b/g? you can make it look more natural by taking a small brush and blurring it lightly around the edges
