It can be as simple as just changing the colors, fonts and adding the team logo, but if you wanted custom trims and patterns that's not that hard either. You can either find a pre-made pattern and drop it inside the main folder that contains the jersey mask or use photoshop's custom shapes and selections to build the patterns.
For example the patterns in the jersey below was created by using the rectangular selection tool to create square blocks on separate layers. I then use the oval selection tool to cut out the slopes to make it look like mountains. I then added a gradient effect to it. All in all it took me more time to think of doing that then actually doing it.
