sad, looks like an old wound or a birth defect, he can probably still stick out his tongue to get the nectar though

Probably has a fungus or some sort of disease of the beak.

His tongue sticks out like that in every frame... he guards the feeder fiercely.

They fly pretty fast, kinda looks like he does not always look where he is going :)
