I don't know the answers, but I guess you're not going to get many replies because it's so dependent on where in those countries you are. Cigarette prices are fixed, in all three I think (memory tells me thy're E3.25 in Italy, about the same in Spain, closer to E4 in Germany, but it's been a couple of months). And I haven't been to any of those countries reently enough to give you an answer.
