Mostly with the diffuse glow filter in photoshop (Filter->Distort->Diffuse Glow). If there aren't enough whites, duplicate the layer and change the blending mode to screen and with some mask work get more whites in the areas that you want after you have already applied the diffuse glow. If you want more blur too, apply some gaussian blur on your main subject as well.
edit: One important note: Before applying the diffuse glow filter, you have to make sure that your foreground color is black and your background color is white, otherwise you will get the reverse effect (it will darken the picture instead of brightening it). Simply hitting the letter D on your keyboard in photoshop will take care of the correct foreground/background for you.
